Initial Consultation

Let’s get started.

The first step is to tell us a little about your child. This gives Dr. Ken the information he needs to understand your child’s history and prepare for your consultation.

Step 01

Tell us about your child

Complete a short Google Form with information about your child’s health history, current concerns, medications, supplements, and other relevant details.

This helps Dr. Ken prepare for your consultation.

This form is required before booking your appointment.

Step 02

Choose your appointment

After submitting the intake form, choose an available date and time for your consultation with Dr. Ken.

You’ll be taken to our secure Google Calendar appointment schedule.

Step 03

Send available test results

After booking, please email us any available blood laboratory test results so Dr. Ken can review them before your consultation.

If you don’t have recent blood laboratory test results, that’s fine — you can still proceed with your appointment.
