Steps to Send Laboratory Tests to ArminLabs

STEP 1

Fill out the ArminLabs form using the PDF Laboratory Tests Prescribed by Dr. Ken (you can download the form below)

STEP 2

Attach the completed file to an email addressed to info@arminlabs.com. In the subject line of the email, write Subject:

Request for Laboratory Tests Prescribed by Dr. Ken Alibek

STEP 3

Wait for a reply to your email — a staff member from ArminLabs will contact you with further instructions.
